About the Role

Sevita is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Healthcare & Medical Services team and support individuals in home and community-based care settings. As an LPN with Sevita, you will provide person-centered nursing care to children, adults, and individuals with complex medical, behavioral, or rehabilitative needs, helping them live as independently and safely as possible. Working under the direction of a Registered Nurse and in collaboration with an interdisciplinary team, you will perform hands-on clinical care such as medication administration, treatments, injections, wound care, vital sign monitoring, and documentation in accordance with physician orders and state regulations. You will observe, assess, and report changes in condition, contribute to care plans, and play an active role in health education for individuals and their families. In this role, you will support individuals in a variety of settings, including homes, community residences, and program locations, promoting dignity, choice, and quality of life. You will use your critical thinking and clinical judgment to respond to health concerns, follow established protocols, and escalate issues appropriately.
