About the Role
Signature HealthCARE is seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our Healthcare & Medical Services team. In this role, you will provide resident-centered nursing care to individuals in a long-term care and post-acute environment, supporting their physical, emotional, and psychosocial well-being. The Licensed Practical Nurse will be responsible for delivering high-quality clinical care under the direction of registered nurses and physicians. You will perform hands-on nursing tasks such as monitoring vital signs, administering medications and treatments, providing wound care, and assisting with activities of daily living while maintaining strict adherence to infection control and safety standards. You will accurately document assessments, interventions, and patient responses in the electronic medical record, ensuring clear, timely, and complete clinical documentation. Collaboration is central to this position. You will work closely with an interdisciplinary team that may include RNs, physicians, nurse practitioners, therapists, social workers, and dietary staff to develop and implement individualized care plans. As a key advocate for residents, you will participate in care conferences, communicate changes in condition promptly, and help coordinate services that promote optimal outcomes and quality of life. You will build trusting relationships with residents and their families, providing education, emotional support, and clear explanations of care plans and treatment options. The LPN will help prevent falls, pressure injuries, and infections by following established care pathways and safety policies. You will support a dignified and respectful environment for residents, promoting independence to the highest level possible while providing compassionate assistance when needed. This role requires strong clinical judgment within the LPN scope of practice, excellent time management skills, and the ability to prioritize multiple resident needs.
