About the Role
CarePartners is seeking a compassionate and clinically skilled Licensed Practical Nurse (LPN) to join our Inpatient Rehab Hospital team within the Healthcare & Medical Services department. In this role, you will provide direct, hands-on nursing care to adult and geriatric patients recovering from illness, injury, or surgery in a rehabilitation setting, supporting safe recovery and a successful transition back to the community. Working under the direction of a Registered Nurse and in collaboration with physicians, therapists, case managers, and other members of an interdisciplinary team, you will perform patient assessments within LPN scope, administer medications, monitor vital signs, assist with personal care and mobility, and promptly report changes in patient condition. You will support individualized care plans focused on maximizing functional independence, reinforcing therapy goals, and educating patients and families about ongoing care, medications, and discharge instructions. Consistent with CarePartners’ focus on community health improvement, collaboration, and patient-centered care, you will foster a supportive, respectful environment that prioritizes safety, dignity, and clear communication. Daily responsibilities include accurate and timely documentation in the electronic health record, participation in team huddles and care conferences, coordination of care across disciplines, and adherence to infection prevention and hospital policies. You will help maintain a clean, organized, and safe unit, follow fall-prevention and skin-integrity protocols, and participate in quality improvement initiatives designed to enhance patient outcomes and the overall patient experience. This position is well suited for an LPN who is detail-oriented, comfortable in a fast-paced inpatient rehab environment, and motivated to build strong therapeutic relationships with patients and their families. Candidates should demonstrate solid clinical judgment, strong time-management skills, proficiency with basic computer and electronic medical record systems, and the ability to work rotating shifts, including some weekends and holidays, to support 24/7 inpatient operations.
