About the Role

Ontario Center, a specialized healthcare and rehabilitation provider in Ontario, NY, is seeking a compassionate and dedicated Licensed Practical Nurse to join our Healthcare & Medical Services team. In this role, you will play a key part in delivering high-quality, person-centered care to a diverse population of residents and patients, many with complex medical and rehabilitation needs. Working under the direction of a Registered Nurse and in collaboration with an interdisciplinary care team, you will help ensure that each individual’s care plan is followed, monitored, and continuously optimized.

As a Licensed Practical Nurse at Ontario Center, you will provide direct bedside care, administer medications and treatments, and closely monitor residents’ conditions. You will perform tasks such as obtaining and documenting vital signs, assisting with wound care and dressing changes, supporting respiratory and tracheostomy care as appropriate, and ensuring accurate, timely documentation in the electronic medical record. You will play an important role in observing changes in condition, communicating concerns to the RN and providers, and helping to coordinate appropriate interventions that support safe and effective care.

You will provide resident and family education on medications, treatment plans, and daily care routines, helping them better understand health conditions and participate in decision-making. You may assist with admissions, transfers, and discharges, contributing to smooth transitions of care and a positive experience for residents and their families.

Candidates must hold an active, unencumbered LPN license in the state of New York and current Basic Life Support (BLS) certification. Experience in long-term care, subacute rehab, or a similar healthcare and medical services environment is preferred, though dedicated new graduates with solid clinical training will also be considered.
