Licensed Practical Nurse

CarePartners seeks a compassionate Licensed Practical Nurse to support coordinated, patient-centered care within our Connecticut provider network. The LPN delivers direct bedside care, administers medications and treatments, monitors vital signs, and documents thoroughly in EHR systems. In collaboration with RNs, physicians, and care managers, you help execute care plans that improve community health outcomes. You’ll educate patients and families, support smooth care transitions, and promote safety and infection control, reflecting our values of collaboration, compassion, and high-quality, accessible care for all members.
