About the Role
CarePartners seeks a compassionate Licensed Practical Nurse to support coordinated, patient-centered care across our Connecticut provider network. In this role, you will deliver hands-on nursing care, including assessments, vital signs, medication administration, and basic procedures, while ensuring accurate electronic documentation and regulatory compliance. You’ll collaborate closely with physicians, care managers, and community partners to improve member health outcomes, provide patient and family education, and support outreach and follow-up efforts that strengthen our mission of better, more connected healthcare services.

- LPN licensure
