Licensed Practical Nurse

CarePartners of Connecticut seeks a compassionate Licensed Practical Nurse to support coordinated, patient-centered care across our provider network. In this role, you’ll deliver direct clinical support, administer medications, monitor vitals, and document care using EHR systems. You will educate members on treatment plans and prevention, assist with care coordination and outreach, and collaborate closely with physicians, RNs, and care managers.
