Hawaii Licensed Intake Coordinator
About the Role
A company is looking for an Intake Coordinator - Social Worker. Key Responsibilities Conduct initial screenings and collect demographic and clinical information from prospective patients Coordinate and schedule initial assessment appointments with clinical providers while maintaining patient records Provide support and guidance to patients throughout the intake process and advocate for their needs with external agencies Required Qualifications Bachelor's degree in Social Work (BSW) or applicable Bachelor's degree; or Associate's degree in Social Work with a certificate in substance abuse treatment 1-3 years of post-licensure experience in case management or community-based social work, preferably in addiction or public health Current or license eligible, unrestricted Social Work license in the State of Hawaii (LBSW required) Strong knowledge of community resources and social service systems in Hawaii Proficiency or willingness to learn in using electronic health records (EHR) systems and telehealth platforms
