Home Health Speech Therapist

CenterWell Home Health seeks a Home Health Speech Therapist to deliver patient-centered speech, language, and swallowing therapy in the home setting. In this role, you will perform comprehensive evaluations, create and implement individualized care plans, and educate patients and caregivers to maximize safety, independence, and quality of life. You will collaborate closely with an interdisciplinary team, maintain accurate clinical documentation, and uphold high standards of compassion, integrity, and clinical excellence while contributing to continuous quality improvement.
