Home Health Speech Language Pathologist

CenterWell Home Health seeks a Home Health Speech Language Pathologist to deliver compassionate, in-home therapy to adult and geriatric patients. In this role, you will perform comprehensive speech, language, cognitive, and swallowing evaluations, create individualized care plans, and provide evidence-based treatment to enhance communication, safety, and quality of life. You’ll educate patients and caregivers, document in a home health EMR, and collaborate closely with an interdisciplinary team committed to patient-centered, high-quality care and continuous improvement.
