Home Health Speech Language Pathologist

CenterWell Home Health seeks a compassionate Home Health Speech Language Pathologist to deliver patient-centered therapy in the comfort of patients’ homes. In this role, you will perform comprehensive speech, language, cognitive, and swallowing evaluations; develop evidence-based, individualized treatment plans; and educate patients and caregivers to promote safety, independence, and quality of life. You will collaborate closely with an interdisciplinary team, maintain accurate documentation, and uphold CenterWell’s values of compassion, integrity, and excellence while achieving outstanding patient outcomes.
