About the Role

Medical City Healthcare is seeking a Home Health RN Clinical Manager to lead our growing home health clinical operations within the Healthcare & Medical Services department. In this role, you will combine strong clinical expertise with hands-on leadership to ensure that our patients receive safe, high-quality, and compassionate care in the comfort of their homes.

As the Home Health RN Clinical Manager, you will oversee the daily clinical activities of our home health team, including RNs, LPNs, and aides. You will be responsible for supervising patient assessments, developing and reviewing individualized plans of care, coordinating services, and ensuring that all care is delivered in accordance with physician orders, evidence-based practice, and regulatory standards. You will monitor clinical documentation, support accurate and timely completion of OASIS and other required records, and help drive best practices in electronic medical record (EMR) use.

You will serve as a clinical resource and mentor for field staff, providing guidance on complex patient situations, escalation of concerns, and critical decision-making. This position plays a key role in hiring, onboarding, and ongoing training of clinical staff, fostering a supportive environment that encourages professional growth, accountability, and teamwork. You will assist with scheduling and staffing needs, support performance evaluations, and help implement corrective action plans as needed to maintain a high-performing team.

The Home Health RN Clinical Manager will collaborate closely with physicians, therapists, social workers, and other interdisciplinary team members to coordinate care, prevent avoidable hospitalizations, and promote optimal patient outcomes. You will regularly review quality and outcome data, identify trends or areas for improvement, and implement quality improvement initiatives to elevate patient satisfaction, safety, and clinical effectiveness. Monitoring infection control practices, medication management, wound care standards, and chronic disease management protocols will be central to your oversight.

Regulatory and accreditation compliance is a critical aspect of this role. You will help ensure adherence to Medicare/Medicaid conditions of participation, state and federal home health regulations, and internal policies and procedures. You will participate in chart audits, prepare for and support external surveys, and contribute to the development and revision of clinical policies and standardized processes.

This position requires frequent field presence with the flexibility to work both in the office and remotely for administrative and documentation review. You will communicate regularly with patients and families, addressing questions and concerns, setting expectations about the plan of care, and promoting patient and caregiver engagement in health management. Strong organizational skills, clinical judgment, and the ability to balance competing priorities are essential for success.

The ideal candidate is a Registered Nurse with substantial home health or acute care experience, proven leadership capabilities, and a passion for coaching others. You should be comfortable working with data, clinical metrics, and EMR systems, and be motivated to drive continuous improvement in a dynamic care environment.
