Home Health Physical Therapist Assistant
About the Role
CenterWell Home Health is seeking a compassionate Home Health Physical Therapist Assistant to deliver high-quality, skilled therapy services in patients’ homes. As part of our Healthcare & Medical Services team, you will work under the direction and supervision of a licensed Physical Therapist to help patients regain function, improve mobility, and maximize independence in the comfort of their own environment. You will visit patients in their homes, carry out individualized treatment plans, provide therapeutic exercises and gait training, and instruct patients and caregivers in safe mobility, use of assistive devices, and home exercise programs. You will carefully observe and document patient progress, communicate changes in condition to the supervising Physical Therapist and clinical team, and contribute to updating and adapting the plan of care to support optimal patient outcomes. This role requires strong clinical skills, sound judgment, and the ability to work independently in the field while remaining highly collaborative with an interdisciplinary care team. You will practice in alignment with CenterWell’s values of compassion, integrity, and excellence, maintaining strict adherence to safety standards, infection control, and regulatory and documentation requirements.
