About the Role
Oak Street Health is seeking a compassionate and mission-driven Primary Care Physician to join our Healthcare & Medical Services team as a Primary Care Physician-OSH. In this role, you will deliver high-quality, comprehensive primary care to an adult Medicare population, with a strong focus on underserved communities. You will manage a dedicated patient panel, emphasizing preventative care, chronic disease management, and coordination of services to improve health outcomes and reduce avoidable hospitalizations. As a Primary Care Physician at Oak Street Health, you will practice in a value-based, team-oriented environment. You will collaborate closely with nurse practitioners, nurses, medical assistants, care managers, social workers, and other interdisciplinary team members to provide patient-centered, holistic care. Your typical day will include performing patient assessments, conducting routine and complex primary care visits, developing and updating care plans, ordering and interpreting diagnostic tests, prescribing medications, and providing patient education to empower individuals in managing their health. You will play a key role in care coordination by communicating with specialists, hospitals, and community resources to ensure continuity and quality of care. You will use electronic health records and population health tools to track patient outcomes, close care gaps, and proactively reach out to high-risk patients. You will also participate in quality improvement initiatives, data-driven case reviews, and team huddles to enhance the effectiveness and efficiency of care delivery.
