Case Management Worker (lsw/lcsw)

Carle Health is seeking a dedicated Case Management Worker (LSW/LCSW) to join our Community & Social Services team within our Healthcare & Medical Services network. In this role, you will provide patient-centered case management and social work services that support individuals and families across the continuum of care, with a strong focus on community-based resources and improving health outcomes.
The Case Management Worker (LSW/LCSW) will conduct comprehensive psychosocial assessments to identify patient needs, strengths, and barriers related to medical conditions, mental health, social determinants of health, and family dynamics. You will collaborate closely with physicians, nurses, therapists, and other members of an interdisciplinary care team to develop and implement individualized care and discharge plans that promote safety, continuity of care, and optimal patient outcomes.
This position involves coordinating access to community resources, social service agencies, financial assistance programs, behavioral health supports, and other relevant services. You will provide counseling, crisis intervention, and supportive case management to patients and families dealing with complex medical issues, life transitions, and psychosocial stressors. As an advocate for patients and families, you will help them navigate healthcare systems, understand their care options, and connect with services that address both immediate and long-term needs.
The Case Management Worker will document assessments, interventions, referrals, and outcomes in the electronic health record (EHR) in a timely and accurate manner, adhering to professional, regulatory, and organizational standards. You will participate in case conferences, interdisciplinary rounds, and team meetings to share updates, refine care plans, and support coordinated, high-quality care. In addition, you will contribute to quality improvement initiatives aimed at enhancing patient safety, care transitions, and access to community-based support.
